BMI Limitations and Canadian Health Guidelines

Body Mass Index was developed by Belgian statistician Adolphe Quetelet in the 1830s as a population-level measure, not a diagnostic tool for individuals. Health Canada uses BMI as an initial screening indicator, but recognizes its significant limitations: it cannot distinguish between muscle and fat mass, does not account for bone density or body composition, and may misclassify athletes, older adults, and certain ethnic groups. A muscular individual with 12% body fat could be classified as “overweight” by BMI alone.

The Canadian Guidelines for Body Weight Classification in Adults recommend using BMI alongside waist circumference for a more complete health risk assessment. Abdominal fat is a stronger predictor of cardiovascular disease and type 2 diabetes than BMI. Health Canada defines elevated risk as a waist circumference exceeding 102 cm (40 inches) for men and 88 cm (35 inches) for women. For individuals of South Asian, Chinese, or Japanese descent, lower waist circumference thresholds apply due to higher metabolic risk at smaller measurements.
